I am a 53-year-old male and exercise regularly. I was not always so fit but I have lost my beer belly. I would like a tummy tuck. Would I have to take time off work? Is this procedure done at an outpatient clinic?

Answer:
You most certainly would have to take some time off from work. How much time depends on what job you perform and your willingness to refrain from medications after the procedure. In addition, we cannot predict with certainly how each patient will recover from the procedure, however most patients can go back to sedentary type work after about two to three weeks. You would not be back to normal exercising activity for six weeks. This procedure is done at an outpatient surgery center.

I get yeast infections underneath my breasts in hot weather. My bra digs into my shoulders and I get back aches almost daily. Exercising vigorously is embarrassing. All this is because I was cursed with large breasts. I would like breast reduction surgery. My areolas are abnormally large and I would like to know if I can get them reduced in size too.

Answer:

Absolutely. Areola reduction is part of the breast reduction surgery. Bilateral breast reductions are one of my favorite procedures because the patients get almost immediate symptomatic relief.  In addition, not only do they feel better but they look better as well. The surgery takes approximately six weeks to recover in terms of being able to go back to all activities such as heavy exercising after this procedure.  

There are different forms of breast reduction surgery and the scars can be limited to just around the areola as a circle; around the areola and down the breast which is the lollipop incision; or finally as an anchor incision which is incision and scarring that has been around the longest. In my practice I primarily use the lollipop breast lift or breast reduction, which is the scar all the way around the areola and down vertically to the bottom of the breast. You should consult with a Board Certified Plastic Surgeon who can determine which of these procedures is best for you.  It is important to know that each patient has individual characteristics which must be discussed with the surgeon.

I am 48 years old and quite active socially. I know men are more attracted to women who have larger breasts and I would like implants. Are breast implants noticeable?

Answer:
First of all, I do not totally agree with your statement that men are attracted to women who have larger breasts. I do not think that this should be a reason that you undergo a breast augmentation. I feel that you should want them for your own sake. Yes, I can understand your wanting to appear attractive for an increased sense of self esteem and self confidence, but I do not think that you should undergo a surgical procedure just to please somebody else.

Having said that, the answer to your question is not all breast implants are noticeable. The naturalness of the result depends upon two conditions. First, how much breast tissue you had to begin with and second how big you want to be. If you have a lot of breast tissue and you only want to increase your size moderately, then the breast implants will be fairly unnoticeable. If you have very little breast tissue or no breast tissue and you want to be very big, the end result is going to be almost entirely breast implant which will be noticeable. You should consult with a Board Certified Plastic Surgeon who can go over all the alternatives of breast augmentation with you and take you through the procedure from start to finish.

Can breast augmentation be done using my own body fat?  I worry about the implants leaking.

Answer:
Breast augmentation by fat injections alone has not caught on to a large extent in the United States.  Very few doctors are willing to do breast augmentation by this method alone as it probably takes multiple injections and there are some medical/legal issues because we are not exactly sure how fat injections over the course of your lifetime will affect a mammogram.  Because fat injections could potentially cause some micro-calcifications on the mammogram, this could be confused with a very early breast cancer.  Many surgeons do advocate fat injections for breast augmentation, however more than one treatment is necessary and there are some potential side effects and complications that you should know about.

I am a 24-year-old man and despite using protein drinks and weight lifting I cannot increase the size of my triceps. Do you suggest fat grafting to bulk up this area?

Answer:
I have no personal experience using fat grafting to bulk up the triceps area and my initial thought is that it should not be done.  You would require a lot of fat to make a noticeable change and it would not look very natural as the triceps area is an area that contracts and looks differently with different arm positions.  Fat grafts would not contract like normal muscle and for that reason I do not think it is the best alternative. My suggestion would be to continue with your diligent workouts and diet.  I am confident that with continued workouts and diligence that you will see an improvement in the definition of your triceps muscles.

Your Photo Gallery of before and after pictures of liposuction is helpful. I can really see what to expect. I am interested in Liposculpture using laser. Does this result in much scarring?

Answer:
The scarring from Liposculpture using laser is very minimal. The incisions that are used to access the subcutaneous fat and the under surface of the skin are very small, from between 2 mm to 0.5 cm. These should fade with time and become very inconspicuous, however there is no guarantee that this will occur.  Occasionally, even though these scars are very small they can become red or widened or bumpy.  This, however, can be treated easily if that does occur.

Do you recommend a second opinion when going for a body lift? What dangers are inherent in this type of procedure?

Answer:

I certainly would always recommend someone get more than one opinion because there are various ways to approach a particular problem.  Getting multiple professional views should be helpful, although sometimes patients get confused when they are presented with different treatment options. For a body lift I would get two opinions of doctors who specialize in post massive weight loss body contouring.  The dangers inherent in this type of procedure are most related to anesthesia when multiple procedures are undertaken at one time. The most dangerous complications from an anesthetic after body lifts are related to pulmonary complications and cardiac complications and occur in older patients, patients who have underlying medical illnesses or in patients who have a prolonged operative case. In patients who are under anesthesia for long periods of time, the risks for blood clots increases.  The risk of blood clots also increase for those patients who have a history of blood clots, who are obese, or who have underlying medical illnesses such as a history of coronary artery disease or stroke.

You should discuss the anesthetic risks with your plastic surgeon before scheduling any surgery so you are aware of the potential risks. Although these risks can be devastating, the incidence of these complications are very low and so you should not be too concerned about them, however you should be informed. There are other potential complications from body lift surgery such as developing a fluid collection underneath the skin, hypertrophic or widened scars and infections.  Although these are less dangerous than the previously mentioned anesthetic complications, their incidence is slightly higher.

I want a complete lower body lift. Will it be difficult to care for my one-year-old child afterward and for how long?

Answer:
Yes, you should plan on not picking up your child for approximately six weeks. One of the biggest risks of lower body lift is scar migration or scar widening.  This can happen any time early on if you do not take care to avoid heavy lifting for the first six weeks after surgery. Even with precautions, there still is a chance that the scars may not be acceptable, however the more compliant you are, the more likely your scars will be acceptable.

I am a 37-year-old profession female body builder so I have to look good in a bathing suit. My buttocks, despite years of working on them, are almost pure muscle but are too flat. Would a body lift improve the shape of my buttock?

Answer:

A body lift is a term used to describe lifting the entire lower body, meaning the abdomen, back and thighs.  This would achieve some lifting of the buttock area, however this is way too large of a procedure to achieve only the goal of improving the shape of your buttocks. If the buttocks is your isolated problem then a body lift is certainly not the procedure of choice for your particular problem. There are different buttock lifts that I do not think the trade offs are worth the improvement.  The biggest trade off is scarring and there is always visible scarring when doing a buttock lift unless you hide it in the inferior gluteal crease and that is not a fold where you can lift the buttocks but you can change the shape that way. Other alternatives for you to improve the shape of your buttocks include fat grafting and implants. In my estimation these are two better options for you and you should discuss this with a plastic surgeon.

I swear I have the worst breasts on this planet. I am 25 years old and avoid intimacy and bathing suits and anything that will make me show my breasts. I would like implants, probably saline.  If I chose a certain cup size can more saline be injected into the implant later to enlarge the cup size? I would like to start out with a B cup but I think I might prefer a C cup.

Answer:
There are some implants which you can fill after placement.  Some implants have a port that can be implanted underneath the skin and filled with saline progressively until the desired size is achieved.  The port can then be removed and you can live with the breast size that you desire.  I do not use these types of implants but they are available. Standard regular saline implants could also possibly be inflated more after implantation.  This, however, would require a re-operation as normal saline implants do not have a port that could be placed subcutaneously and then filled.
